We have taken steps to help ensure your time at Your Family Psychiatrist is an enjoyable one. Wait times are minimal as we value your time. To help us to continue to provide these services, please follow the below policies.
Cancellations: Two full business days notice is required to cancel an appointment. Appointments cancelled with less than 2 business days will be charged at the full fee. Cancelling an appointment on short notice or missing an appointment results in lost time when care could have been provided.
Refills: Refills are provided during appointment times. It is the patient's responsibility to schedule the next appointment 1 month in advance to secure an appointment before running out of medication.
Forms: Routine school/work excuses are provided upon request during your appointment time. If time allows, brief forms (<5 min) may be completed during your appointment time without charge. Please notify your psychiatrist if forms/letters are requested at the beginning of your appointment. Longer forms and letters can be completed outside of your appointment time for a fee. The fee is based upon the time involved to complete the service. 5-29 minutes - $200 >29 minutes - $400/hr
Record Requests: Upon written notice, records can be copied. Please allow 1 week to have copies made. Copies can be picked-up or mailed to the patient's address only. No faxes are provided. The fee is $25 for the first 20 pages and $0.50 for each subsequent page.
Phone calls: After-hour non-emergency calls or calls lasting longer than 5 minutes that otherwise would have necessitated an appointment will be charged at $50/5 minutes. In the case of an emergency, please call 911 or go to the emergency room.
Reminders: Active patients consent to receiving emails, texts, and phone call reminders for appointments unless they specifically opt-out.
Counselors: Licensed Professional Counselors Associates are under the direct supervision of a Licensed Professional Counselor Supervisor. The current supervisor is Ryan P Holliman PhD, LPC-S #62571 and can be reached at 214-239-4570 or [email protected].
Patient Relationship: If you have not been seen by a practitioner in over 8 months, your provider-patient relationship has ended. Practitioner-patient relationships may resume once you are seen in our office by a practitioner.
Complaints: We would appreciate notice of any concerns or complaints be sent to [email protected] so that we may improve any deficiencies.
